The Superior Family Health Team is seeking a Physiotherapist who is a dynamic team player to join an innovative, community-based primary health care team.
Working as a Physiotherapist on a multidisciplinary team, you will:
Work collaboratively with other Interdisciplinary Health Professionals to provide comprehensive care plans for our patients
Be responsible for the delivery of patient-centred physiotherapy services
In collaboration with the Primary Care Provider and other members of the interdisciplinary team, develop and discuss appropriate individualized care plan with the patient/family, caregiver based on best practice
Conduct consultations with patient/family to obtain relevant information and assess patient/family learning and needs
Be involved in health promotion, program planning, continuous quality improvement and education for our patients and team members
Be a resource to Primary Care Providers and other allied health professionals
Your profile will include:
A Bachelors Degree is Physiotherapy
Registered with the College of Physiotherapists of Ontario and in good standing
Membership in the Canadian Physiotherapy Association
Ability to work independently and collaboratively within an interdisciplinary health care team
Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills
