NORTH CHANNEL NURSE PRACTITIONER LED CLINIC

On November 29, 2007, the government of Ontario committed to establishing 25 new Nurse Practitioner-Led (NP-Led) Clinics. The clinics are funded by the Ministry of Health and Long Term Care (MOHLTC) as part of their commitment to providing quality health care for all Ontarians.
The North Channel Nurse Practitioner-Led Clinic (NCNPLC) is the 25th clinic to open in the province.
The NC NPLC opened its doors to unattached or orphaned patients on January 29, 2014, with a catchment area spanning from Echo Bay to Iron Bridge.
The Clinic is situated at 135 Dawson Street in Thessalon, Ontario, directly across from Algoma Manor and adjacent to the Thessalon Hospital.
Nurse Practitioner-Led Clinics (NPLCs) are primary health care models in which Nurse Practitioners work collaboratively with an interprofessional team, including a consulting physician, to provide comprehensive, accessible, and coordinated family health care services to people who formerly did not have access to a primary care provider (i.e. unattached patients).
Patients are registered with the clinic to receive primary care on a long term basis, as in other primary care models. All NPLCs are not-for-profit organizations and led by a board of directors.
The NCNPLC delivers comprehensive, patient focused primary health care to clients in the community, holistic care that addresses the needs of each client from birth to senior years, health promotion and disease prevention, chronic disease management, support for persons with mental illness, care coordination and navigation of the health care system and integrated care within the community through established partnerships.
The clinic currently employs on a full time basis, two Nurse Practitioners (NP), a Social Worker (RSW), Registered Nurse (RN), Registered Practical Nurse (RPN), an Administrator and Administrative Assistant.
The board of directors consists of six members, two of whom are NPs.
The NCNPLC held their official Grand Opening on Thursday, October 9, 2014.
The celebration coincides with the Community Health and Wellbeing Week that takes place this year from October 5-11, 2014.
The event included representatives from the MOHLTC, the Association of Ontario Health Centres (AOHC), the Nurse Practitioner Association of Ontario (NPAO), local government and municipal representatives.
The NCNPLC is committed to providing high quality health care to its clients and developing long term, collaborative relationships within their area.
